INITIAL NCOER COUNSELING
1. The purpose of this document is to record the initial counseling of the platoon leaders within UNIT. This initial counseling was conducted on ________, between _________ and ______________.

2. Your principal duties: XXXXXXXXX. Responsible for all personnel assigned in the areas of training, safety, soldier/family welfare, as well as personal and professional development.

3. Your appointed duties include: XXXXXXXXX.

4. PROFESSIONALISM. In addition to your principal duties, the most important responsibility you have as a leader is to set a proper professional example. Especially important are the ethical values of loyalty, duty, selflessness, and integrity. Failure to meet standards in these areas indicates a basic flaw in your character and may precipitate a relief for cause.

a. Selflessness. You must place the goals and missions of the unit above your personal goals. You must be dedicated to their accomplishment in spite of their impact on your personal welfare or comfort.

b. Pride/Team. You must show pride in what you, your squad, platoon, company, and battalion do. Once committed to executing a mission, you must give 100% and maintain a positive attitude. Always remember that your squad or platoon is part of a team and that what you may have to do may not always be best for your squad or platoon, but someone thinks it is best for the team. When you get to choose what you will do, think about what would be good for the unit.

c. Orders. Execute your orders as completely and rapidly as possible. Obey the spirit of the order, not just the letter of it. Execute implied tasks just as faithfully as specified tasks; you will be held responsible for both.

d. Missions. I assume all missions given to you will be accomplished. If your mission is in danger of failing, tell me ASAP so we can adjust our plans.

e. Honesty. Be absolutely honest in your communications with me and with your subordinates. Cheating during training or falsifying documents will never be condoned.

f. Conduct. Be a good example of the American Soldier both on and off duty. Drink prudently and do not abuse alcohol; under no circumstances should you drink and drive. Stay in control of your finances and don't overextend yourself.

g. Take Responsibility. Do the harder right, not the easier wrong. It does not matter what everyone else did, or didn't do, you will be held responsible for the decisions you make. Be willing to stand-alone for what you know is right against your peers, your subordinates, or me.

5. PERFORMANCE. The principal duties as discussed in paragraph 3 above are performance oriented. It is expected that you will perform all of your duties to meet and exceed the Army and unit standards. Don't wait to be told to do something you know needs to be done. Take initiative and don't be afraid to make a mistake. I anticipate that you will fail to meet standards from time to time, and this will result in an opportunity for retraining. However, a pattern of substandard performance after opportunities to retrain indicates either an inability or unwillingness to meet the standard. If the standard is sound, an inability or unwillingness to meet it may be cause for relief.

a. Competence is the key to accomplishing your duties. Do all you can to develop yourself into a great NCO and soldier.

b. Physical fitness and military bearing will go a long way toward communicating that you are a competent professional. Always stay fit and be sharp.
(1) You will pass the APFT at least twice a year. The goal for squad leaders and platoon sergeants is 270 or above. You will also meet the proper height-weight standards.
(2) Always present yourself as well groomed and always meet Army haircut standards. You will shave first thing every morning to include in the field. You will wear all of your uniforms properly and maintain a sharp appearance.
(3) Display mental and physical stamina. Never give up. The standard is you will try and try again, then seek help. The goal is that you will always attempt to complete the mission unless told to stop.
(4) You will act energetically and will communicate clearly and with confidence. Never complain or criticize other officers/NCO's in front of soldiers.

c. Leadership is what the Army pays you for. To lead, you must know where you are going. Therefore, define your objective, then get your squad or platoon there.
(1) Always put the mission first. If you or your squad or platoon stop trying, you have failed. The goal is 100% mission accomplishment, IAW published standards, without unnecessary energy or resources expended. Also instill in your subordinates the desire to achieve and accomplish the mission.
(2) Lead from the front. You will try the hardest, work the longest, get the dirtiest, be the coldest, and LEAD your soldiers.
(3) Take care of your soldiers. Be more concerned for their welfare than your own and ensure they know it. Take care of your soldiers' families. The Army is a lifestyle, and not just a job, know how to influence lives.
(4) In discipline, be strict but fair and remain consistent. If you bring a soldier to me, expect fair but strict punishment.
(5) Give clear guidance (task, conditions, and standards) to your subordinates so they can accomplish their mission. Avoid "yesterday management."
(6) The Commanders, 1SG's and XO's orders are mine - comply with them. Utilize their knowledge and experience to your advantage.
(7) Be a leader not a hero. A leader makes hard, unpopular decisions, while a hero decides on what the population wants. Soldiers like their heroes, but they respect their leaders.
(8) By virtue of your position you are owed the respect of your subordinates; despite this, you will make every effort to earn their respect. Don't make them earn your trust, give it to them and make them prove themselves unworthy. Most soldiers will appreciate this trust and will fight to keep it. Don't make a superior fight to earn your respect. It is owed to him or her and he or she should have to prove to be unworthy of it.

d. Training is our #1 priority. You will use your abilities and resources to train your squad or platoon. Always remain ready to fight anywhere, anytime. Never waste an opportunity to assess the performance of tasks in training.
(1) Understand training management as outlined in FM 25-101 and make it work when you train. Maintain a current squad or platoon collective task assessment.
(2) Conduct tough and realistic training. It is your responsibility to ensure all your squad's or platoon's training is well planned and thoroughly resourced. Training time is precious. Every time you train, it will be oriented on METL supporting tasks and Battle drills we determine to be most important.
(3) Train your soldiers on the things that will make them successful. If they do not qualify with their weapon and pass PT tests, you as their trainer will be held responsible for getting them there.

e. Responsibilities. For you to be a useful and dependable squad leader or platoon sergeant, you must be held responsible for all of your duties, your soldiers and their actions, your equipment, and any other Army facilities you have influence over. You must hold your subordinates accountable in the same way.
(1) Take care of the equipment and facilities you are responsible for. Keep them clean and serviceable. Get them fixed when they break. Do not lose things. When you recognize someone has mistreated, broken, or lost something, be the bad guy and hold him or her responsible for it.
(2) Conserve our limited training resources. Do not waste supplies.

f. Safety. Ensure that safety is foremost in your and your subordinates' minds. Preach it often, but don't become paralyzed by the fear of an accident. You live in a hazardous environment and must know how to safely operate in it. In whatever you do, conduct a thorough risk assessment and devise specific risk reduction measures to prevent accidents. The goal is that your subordinates do this on their own. The standard is that no soldiers lose life, limb, eyesight, or have irreversible damage done to them that you could have prevented.

g. Remember that you are accountable to both me and to your senior rater. Both of us should know what you and your squad or platoon are up to at all times. Hold your subordinates responsible to stay in touch one up and one down at all times. Check in with me prior to PT and before you and your squad or platoon leave for the day.

6. I have attempted to be specific, but there are still many facets we have not yet discussed. Communicate with me regularly, so that we both know where you stand overall. We will assess and discuss your performance during monthly and quarterly counselings.

APFT FAILURE COUNSELING
On ____________, you failed the semi-annual Physical Fitness Test. IAW AR 350-15, Chapter 1, you are required to take and pass the APFT twice each year. You are being counseled so you may clearly understand the serious actions which may be initiated against you for failing to achieve a passing score on each of the three APFT events.

_______________Effective immediately you are considered a PT failure. You are informed that Report for Suspension of Favorable Personnel Actions has been completed. Your personnel records are now flagged and you are ineligible for promotion and reenlistment. Your records will continue to be flagged until you pass the APFT. All PT failures will be given a "FOR THE RECORD" test every 90 days until successfully completed. Your records will be flagged until you pass the APFT. In addition to attending regularly scheduled PT formations, you will attend REMEDIAL formations at ____________________. Continued unsatisfactory conduct/performance cannot and will not be allowed to continue. Should you continue to regularly fail the APFT without any medical justification, you could be processed for elimination from the service under Chapter 13 of AR 635-200. If conduct worsens, you could be discharged for misconduct under Chapter 14, AR 635-200, resulting in an other than honorable discharge. If separated for your duty performance, the least favorable character of discharge you could receive is a general discharge. If separated for misconduct, you could receive an under other than honorable conditions discharge. Both may cause undue hardships in civilian life and may reflect on your eligibility for veteran's benefits, eligibility for reentry into the Armed Forces, and acceptability for employment in the civilian community.

BAR TO REENLISTMENT
On ________________, a Bar to Reenlistment was initiated in accordance with AR 601-280, Chapter 6, for _________________.

On ______________, under the provisions of AR 601-280, paragraph 6-5e, I am informing you that you have 7 days from this date to summit an appeal to the Bar to Reenlistment. Your appeal, if you decide to submit one, must be returned to me by ______hrs, on _____________. On case by case basis an extension of this period may by granted. Appeals will be endorsed personally by each commander (or acting commander) in the chain of command. For soldiers with less than 10 years active Federal service at ETS, the approval or disapproval authority is the Commanding General of Ft. XX. For soldiers with more than 10 years active Federal service at ETS, the approval authority is the Commander, MACOM. You are also informed the bar will be reviewed every 6 months or 30 days prior to your PCS or ETS. Mandatory separation proceedings will be initiated if you fail to overcome the bar by the completion of the second 6 months of review. If you believe you will be unable to overcome the bar to reenlistment you may apply for immediate discharge IAW para 6-5f, AR 601-280 and under the provisions of AR 635-200, Chapter 16. You are now in a non-promotable status IAW para 7-6, AR 600-200. I will forward a recommendation to lift this bar to reenlistment anytime I believe your duty performance warrants retention.

CIVILIAN EDUCATION COUNSELING
Congratulations on enrolling in _______________.Your personal initiative to improve your educational level through appropriate academic/vocational schooling reflects your concern for your future. This is an important step toward furthering your self development and will ultimately enhance both your personal and professional endeavours. I appauld and support this decision.
Receipt of tutition assistance dictates that your place of duty during class is the classroom. Failure on your part, for non-military commitments, to attend this course may result in UCMJ action, loss of educational benefits, or both. However, be advised that course enrollment does not automatically guarantee class attendance. Your ability to attend class during assigned duty hours will be based on current and future operational commitments and manning levels. In the event that you are unable to attend class due to operational/manning requirements, we will notify your instructor so that your abscense will not have a negative impact on your pursuit.

CORRECTIVE TRAINING and COUNSELING

3. Corrective training (extra or remedial training) is an effective non-punitive disciplinary measure available when a soldier's duty performance has been deficient and the soldier would benefit from extra training. It must be directly related to an observed deficiency and it must be oriented towards improving the soldier's performance in the problem area.
4. A soldier's commander or any NCO in the soldier's supervisory chain may authorize corrective training. A member of the soldier's chain of command should be present and supervising the corrective training session.
5. Army regulations allow corrective training to be conducted during or after normal duty hours.
6. Corrective training is not punishment and should not be confused with extra duty imposed as punishments under Article 15, UCMJ. Extra duty need not be related to the soldier's misconduct. For example, a soldier who receives an Article 15 because he was AWOL may be required to perform cleaning duties as extra duty. Extra duty imposed as punishment under Article 15, UCMJ may be ordered executed during any off-duty time.

DEBT OBLIGATIONS
The Army requires that soldiers pay their debts promptly, failure to do so damages credit reputations and effects the Army's public image. The willful failure to resolve unpaid debts may result in administrative or punitive actions to include:
a. Making it a matter of permanent record by the placement of memorandum of reprimand, admonitions, or censure in
the soldier's official personnel files.
b. Denial of reenlistment under AR 601-280.
c. Administrative separation from the service (AR 635-100 or AR 635-200).
d. Punishment under the UCMJ, when proper such misconduct may be charged under Article 92, 123, 133, or 134 of the
UCMJ.
Counseling service is available under the Legal Assistance Program (AR 27-3). You are urged to seek budget counseling and consumer protection advice. These services may be obtained from on-post credit unions, Army Community Service Program Counselors (AR 608-1), or through financial management seminars or workshops. You may write your intentions about the debt in block 11 of the DA Form 4856-R. The impact of an adverse separation can be severe depending upon the type of discharge you receive. A dishonorable discharge deprives a soldier of all veteran's benefits and may deprive him or her of their civil rights. If separated for your duty performance, the least favorable character of discharge you could receive is an under other than honorable conditions discharge. Both may cause undue hardships in civilian life and may reflect on your eligibility for veteran's benefits, eligibility for reentry into the Armed Forces, and acceptability for employment in the civilian community.

LOST MILITARY IDENTIFICATION CARD COUNSELING
On ________, you lost your Military Identification Card. Your Military Identification Card is military property. It is your responsibility to report a loss of governtment property immediately to your chain of command. It is your responsibility to maintain positive control of your Military Identification Card at all times. Loss of your Military Identification Card jeoprodizes the security of all military installations worldwide. You are required to report to the Identification Card section to the Provost Marshall and _____________ then have another Military Identification Card re-issued immediately. Separation actions may be initiated if unsatisfactory behavior or performances continues. Unsatisfactory performances and acts or patterns of misconduct can result in separation from the Army under AR 635-200. Chapter 13 ( Unsatisfactory Performance ) is the commanders judgement of your performance. Chapter 14 (Misconduct) is misconduct because of minor disciplinary infractions, a pattern of misconduct, commission of a serious offense, conviction by civil authorities, desertion, and absence without leave. Under the provisions of these chapters, discharge from the Army can be characterized as: Chapter 13 Honorable or under honorable conditions as warranted by their military record. Chapter 14: a Discharge under other than honorable conditions is normally appropriate for a soldier under this chapter. However, the separation authority may direct a general discharge if such is merited by the soldiers overall record.

MOTORCYCLE RIDER
The purpose of this counseling is to inform you the requirements for motorcycle safety and licensing requirements.
Per Brigade Commander's Guidance, the Company Commander will visually inspect each rider's motorcycle (to include DoD decal), drivers license, and motorcycle training certificates.
Each motorcycle rider will be required to complete the basic motorcycle course upon notification. Riders are highly encouraged to complere the advanced rider's course.
Army motorcycle riders are responsible for wearing personal protective equipment (PPE) both on and off post. Soldiers that fail to wear PPE off-post and are in an accident will be subject to a line of duty investigation and possibly not receive SGLI or medical benefits from an accident. Higher visible PPE is preferred. Required PPE is a helmet that meets the Dept of Transpo requirements, eye protection, jacket, pants, gloves, boots. Rain suits are not required, but highly encouraged in the
event of adverse weather conditions.
At all times assess your skill level and those that you are riding with. If the other riders are more skilled and experienced, do not attempt to keep up with other riders.
Ensure that you remove DoD decals and return to the post Provost Marshall in the event that you sell your motorcycle.
In the event that you identify other motorcycle riders in the unit, notify your chain of command.
Motorcycles are a safe form of transportation if we take the appropriate actions in ensuring safety and being responsible. Ensure that your motorcycle fits your body and the intent of its use. As with all forms of transportation, drinking and driving will not be tolerated. Those found guilty of drinking and driving will receive a General Memorandum of Reprimand in their permanent file.
If you are a licensed motorcycle rider, but do not own a motorcycle, all of the above applies if you ride a motorcycle that belongs to someone else.

PREGNANCY COUNSELING
Congratulations on your pregnancy. This counseling provides required details . I affirm that I have been counseled by XXXXX this date on all items on the attached counseling checklist and I understand my entitlements and responsibilities. I understand that if I elect separation I may receive maternity care at Department of Defense expense, on a space available basis for up to 6 weeks postpartum for the birth of my child only in a military medical treatment facility which has maternity care capability and that I may elect a separation date no later than 30 days prior to expected date of delivery, or latest date my physician will authorize me travel, whichever is earlier. Further, I understand that many military medical treatment facilities cannot provide maternity care and that unforeseen circumstances or medical emergency could force me to use civilian medical treatment facilities following separation from active duty. Should this happen, I fully understand that UNDER NO CIRCUMSTANCES can CHAMPUS, any military department, or the Veterans Administration reimburse my civilian maternity care expenses. Such costs will be a matter of my personal responsibility. Further, I understand that the separation authority, in conjunction with my military physician and the needs of the Army, will determine my separation date. I also understand that if I should remain on active duty, I will be expected to fulfill the terms of my enlistment contract. If I elect to remain on active duty, I understand that I must remain available for unrestricted service on a worldwide basis when directed and that I will be afforded no special consideration in duty assignments or duty stations based on my status as a parent.
Per AR 635-200:
1. Retention or separation: You may choose to remain in the Service, or separate.
2. Maternity Care: If you remain on active duty, you will receive treatment in a military facility, or in a civilian facility if there is no military maternity care available within 30 miles of your location, per AR 40-3, Para 2-35
3. For care after separation: If you separate, you are authorized treatment only in a military facility which has maternity care. You are NOT authorized care in a civilian facility at Government expense.
4. Leave: You may request ordinary, advance, and excess leave in order to return home, or other appropriate place, for the birth of your child or to receive other maternity care. Such leave usually terminates with onset of labor. Nonchargeable convalescent leave for postpartum care is limited to the amount of time essential to meet your medical needs, per AR 630-5, Cpt 9
5. Maternity and clothing uniforms: Military maternity uniforms will be provided to you, per AR 670-1, Chpt 24
6. BAQ and Government Quarters: Availability depends on the status of quarters at your installation.
7. Assignments: You will not normally receive PCS orders directing movement overseas during your pregnancy. However, you will be considered available for unrestricted worldwide assignment upon completion of post-partum care. If overseas, you remain assigned overseas, per AR 614-30, Para 2-5
8. Separation: If your performance or conduct warrant separation for unsatisfactory performance or misconduct, or if parenthood interferes with your duty performance, you may be separated involuntarily even though you are pregnant, per AR 635-200, Para 5-8, 13-2, & Chp 11, 14
9. Family care: You must have an approved Family Care Plan on file stating actions to be taken in the event you are assigned to an area where dependents are not authorized or you are absent from your home or military duty. failure to develop an approved care plan will result in a bar to reenlistment, per DA Pam 600-8, Pro 9-6, AR 601-280 Chp 6.

Ensure you check the applicable regulations for EXACT forms and procedures for soldiers enrolled in the Overweight Control program.
On _____________, you reported to the Company Training NCO, for a weigh-in. You were weighed and found to be over the screening table weight and were subsequently taped. You were found to be ________ over the maximum allowable body fat of _______ for your age ( ). A DA Form 268, Report to Suspend Favorable Personnel Actions (Flag) was initiated and signed by the company commander effective this date.

On ______________, you are being entered into the company weight control program. A copy of the flagging action has been provided to you. You are strongly encouraged to participate in the company's PT enhancement program, with the primary focus on meeting the standards set forth in AR 600-9. Participants in this PT enhancement program are required to meet ____________
at __________________. You are required to take XX memorandums to the the TMC/Hospital to have medical personnel determine whether your weight control problem is a result of a medical condition. You must show satisfactory weight loss while enrolled in this program. AR 600-9 states that satisfactory weight loss is 3-8 pounds per month. You will remain flagged and enrolled in the company's program until such time as you meet the standards set forth in AR 600-9. Your next scheduled weigh-in is _________________. Failure to make satisfactory progress in the company's weight control program could result in a Bar to Reenlistment, initiation of separation from the service under provisions of AR 635-200, or both.

WEIGHT CONTROL PROGRAM REMOVAL
On _____________, you reported to________ for a body fat analysis. You were taped at____ which is ____below the maximum allowable body fat of _______ for your age ( ).

It has been determined that you no longer exceed the body fat standard. As of this date, you were removed from the weight control program per AR 600-9 para 20f. DA Form 268, Report to Suspend Favorable Personnel Actions (Flag) was removed from your offical records by the company commander effective this date per AR 600-31.
It is IMPERATIVE that you understand that if it is determined that you exceed your screening table weight and the body fat standard precribed in AR 600-9, para 20c within 36 months from this date, the following actions will apply:
1. Within 12 months, without medical reason, you will be subect to seperation from the service under AR 600-9 para 20j.
2. After the 12th month, but within 36 months from this date, without medical reason, you will be placed on the weight control program again and have 90 days to meet the standards set forth in AR 600-9; meeting standards within 90 days will result in removal from the program; failure to do so can result in seperation from the service under AR 600-9 para 20j.
3. If you neet the standards and become pregnant, you will be exempt from the standards for the duration of the pregnancy plus the period of convalescent leave after pregnancy. After medical release, you will be entered into the program again, if you fail to the meet standards IAW AR 600-9 para 20c.
4. If a medical condition is determined to exist, AR 600-9 para 20d will be followed.
As part of removal from the weight control program, you are no longer required to attend remedial physical fitness training. However, you are required to maintain a regular workout program, no less than three times per week and maintain healthy eating habits.
Congratulations on your removal from the weight control program.You have demonstrated that you have the ability to meet Army standards. It is extremely important that you strive to maintain these standards as set forth in AR 600-9. The Army has traditionally fostered a military apperance which is neat and trim. Further, an essential function for day-to-day effectiveness and combat readiness of the Army is for all soldiers to maintain a healthy and physically fit posture. Self-discipline to maintain proper weight discipline and appearance are essential to the Army's success. I am confident that you will continue to meet these standards.
